In
order to participate in athletics, band, dance or cheerleading
during the 2007-2008 school year, you must have a NEW physical
on file.
You can go to your own doctor for your physical,
or students may choose to
have their physicals conducted at school
on the specified districtwide date.
(See below.) This service is only available for
high school
athletes and other UIL participants (such as band, dance, cheerleading, etc.)
In either case, you MUST use these LISD
forms:
Form
1 (athletic participation form)
Form
2 (medical history form)
Form
3 (steroid agreement)
Students
need to fill out the participation form and the history
section on the medical form.
The attending physician will fill out the examination information. Students
do not need to bring
the steroid agreement to their doctor, but
will need to sign it and bring it to school
with the other two
completed physical forms.

EXTRA SERVICES:
An echocardiogram screening will be offered during these physicals. The cost
is $5.
This service is provided by an outside vendor. Please
make checks payable to Kathy Crowel.
An echocardiogram screening can detect hypertrophic cardiomyopathy (enlarged
heart)
and certain underlying heart conditions that can lead to sudden cardiac
death. This screening is not
required for the pre-participation physical examination,
but is strongly suggested and is opened to
anyone, not just athletes. A parent/guardian
must be present to fill out permission forms.
If you plan to have an echocardiogram screening, you must print these additional
forms:

Form 2 (results form)
